CHAP'S Access to Care
The Community Health Assessment Process's (CHAP) web-based database is a collection of information which enables Johnson county residents with no or inadequate health insurance to locate both services and providers in matters of health concerns. Content of information has been categorized for user friendly access and availability.
Eligibility criteria for services varies from agency to agency. Please call for specific information. This Website is not meant to be all inclusive of health care services that are available. Every effort will be made to keep information current.
